It's drool. Most of us do it, most of us don't know we do it, and over enough nights it leaves a mark on more than just the pillowcase.
A clean pillow is partly about comfort and partly about what you're breathing in for eight hours. Here's how to clear the smell out and keep it from coming back.
Understanding the source of the smell
Saliva isn't just water. It carries proteins, enzymes and minerals, and those are what cause the trouble.
When you drool, the moisture soaks into the fabric and then dries. What's left behind is a protein residue, and bacteria on the pillow surface start breaking it down. Their byproducts are the actual smell, which is why it gets worse over weeks rather than appearing overnight.
Sweat compounds the problem. It brings salts and acids that feed the same bacteria, so drool and night sweat together produce a sour, musty note neither would create alone.
Two things make it stick around longer:
Depth. Once moisture passes the case and into the filling, surface cleaning won't reach it.

Sunlight and fresh air. A few hours in a sunny, breezy spot lifts a lot of mild odor, mostly by drying the fabric out properly. The sanitizing claims made for sunlight are overstated, and long exposure weakens fibers, so treat it as freshening rather than cleaning.
Both are holding measures. Neither reaches the filling.
When the smell won't shift
Some pillows are past helping.
If the odor survives a proper deep clean and full drying, it's in the core. Same if the pillow's more than a year or two old and heavily used, or if it's gone lumpy and flat. At that point you're maintaining something that's already finished.
